If you are a member of an insurance company we participate with, we will process all claims for you. With our completely computerized office system all claims are promptly processed. You will be responsible to pay any deductible amount, co-payment, or any other balance your insurance carrier deems as your responsibility. If required by your insurance company, please have a valid referral forwarded to the office prior to your visit. All co-payments are due on the day of service and all balances are to be paid within 30 days.
If we do not participate with your insurance company, you will be provided with an itemized statement that can be forwarded to your carrier(s) for reimbursement.
We participate with Medicare, Medicaid, PeachCare for Kids, AARP, Worker's Compensation, Blue Cross & Blue Shield PPO, Cigna, Coventry, United Healthcare, Tricare and several others.
Cosmetic and non-covered services are due on the day of service. For your convenience we accept Cash, Check, Visa, MasterCard, Discover and American Express.
